Home Care Service Managers – NOC 0651
Due to
increased growth, Helping Hands Personal Care Home Ltd. is hiring 2 Home Care
Service Managers for its Grenfell location.
Home
Care Service Manager will serve as team supervisor to Home Care Aides and residents
in our senior care facilities.
ESSENTIAL
DUTIES & RESPONSIBILITIES:
- Supervise
care facilities and following operating policies and procedures.
- Evaluates
home care program and care facility staff on a regular basis and offer
input to continuously develop procedures.
- Receive
and review all resident/client and Care Aides’ concerns.
- Receive
referrals, as they come in.
- Understand
licensing verification and authorization process.
- Act as
a resource for all Care Aide staff in problem solving for concerns.
- Help
facilitate admission interview and assessment of residents/clients
- Helps
assign staff to care situations consistent with their skill level.
- Advises
Senior Management on the daily resident and home care activities related
to client care provided by aides.
- Develops
safe Care Aide scheduling standards based on the care needs of the
residents/clients
- Adheres
to best practises when performing treatments.
- Provides
residents/clients with the knowledge and assistance to learn appropriate
self care techniques.
- Accurately
records residents’/clients’ condition in the documentation.
- Observes,
records and reports to the appropriate professional the symptoms, reaction
and changes including general physical and mental condition of residents
and clients, signs and symptoms which may be indicative of changes in their
health, and stresses in human relationships between residents/clients and personnel,
and their families and visitors.
- Assists
with the rehabilitation of residents/clients according to the medical care
plan through being aware of and encouraging the interests and special
aptitudes of patients; encouraging residents/clients to help themselves
within their own capabilities in performing activities of daily living.
- Practices
safe moving and lifting techniques and trains and supervises other care
staff to do same.
- Promotes
cooperative effort among Care Aides.
- Assists
residents/clients safely with medications and treatments and trains Care
Aides with same.
- Operates
medical equipment as necessary for patient care and safety.
- Participates
in professional development programs annually.
- Participates
in regular performance evaluations with Senior Management.
- Maintains
a continuing knowledge of competencies related to the Care Aide profession
by participation in conferences, workshops and professional organizations.
- Understands
human relationships between and among patients and personnel, patients and
families, families and personnel; recognizes and understands cultural
backgrounds and spiritual needs, respects the religious beliefs of
individual patients.
- Demonstrates
excellent judgement in referring residents/clients to other health
professionals when necessary.
- Implements
the directives of other health professionals within the context of the
home health setting and instructs Care Aides in the implementation of
same.
- Manages
urgent and emergent situations within existing policies and procedures and
demonstrates even temperament in stressful environments.
KNOWLEDGE,
SKILLS AND ABILITIES
- Knowledge of home healthcare management principles,
practices and procedures.
- Ability to interpret established policies into
operating procedures.
- Ability to create new policies and operating
procedures to benefit the residents/clients and benefit the agency as a whole.,
- Ability to coordinate effectively various functions
and activities for maximum cooperation and efficiency.
- Ability to direct, instruct and advise staff.
- Ability to receive and effectively react to
day-to-day problems presented by staff as well as others. Ability to
effectively use verbal and written communication skills.
- Must have the ability to communicate effectively
with diverse groups of people both verbally and in writing.
- Must be friendly, dependable, professional and have
the ability to support a team.
- Must have knowledge of nursing principles,
procedures and health care sector ethics.
- Must have knowledge of healthcare guidelines
essential to residential care. Must have
the ability to adapt or modify standard practices for home-based, individualized
care of each patient.
- Must have demonstrated knowledge of problem
identification and problem solving in the home setting.
- Must have the ability to work with a team and supervise
Care Aides and provide direction as needed.
- Must be able to work weekdays and weekends, if
necessary. Must be able to work day,
evening, or night shifts as required.
- Must be flexible and open-minded.
- Must be able to use critical thinking skills.
- Must be able to speak and communicate in English
and have excellent written English skills.
- Must be outgoing and outspoken and prepared to be a
strong leader who is comfortable with both praising and correcting the Care
Aids within the agency.
EDUCATION:
Must be
a graduate of a reputable caregiver program and must be certified in CPR.
